OTOVEL(R) 소개

OTOVEL(R)(시프로플록사신 0.3%, 플루오시놀론 아세토나이드 0.025%)은 소아 환자(6개월 이상)에서 황색포도상구균, 폐렴구균, 인플루엔자균, 모락셀라 카타랄리스균, 녹농균으로 인한 고막천공관 급성중이염(acute otitis media with tympanostomy tubes, AOMT) 치료에 사용된다. 시프로플록사신 점이제 및 플루오시놀론 아세토나이드 점이제와 비교해 OTOVEL의 효능과 안전성을 평가하고자, 2건의 3상 다센터 무작위 이중맹검 적극제어 병렬군 시험을 진행했다. 이들 시험에는 고막천공관 급성중이염(AOMT) 소아 환자 총 662명(6개월~12세)이 참여했다. 두 시험의 결과, OTOVEL 치료군은 시프로플록사신과 플루오시놀론 아세토나이드 단독 치료군에 비해 이루(귀 고름)가 멈추기까지 걸리는 시간이 유의하게 짧은 것으로 나타났다. OTOVEL 치료군에서 이루가 멈추기까지 걸리는 중간 시간은 시험 1과 시험 2에서 각각 3.75일과 4.94일이었다.

OTOVEL(R)은 살균 일회용 물약 병 형태로 판매되며, 각 병에는 살균 무방부제 귀약 0.25mL가 들어 있다. Arbor가 OTOVEL의 홍보와 유통을 맡고 있으며, 현재 미국 전역에서 OTOVEL을 판매하고 있다.

고막천공관 급성중이염(AOMT)

급성중이염(acute otitis media, AOM)은 고막 뒤의 중이 부위에서 발생하는 염증으로, 주로 감염이 원인이다. 중이에 감염이 발생하면 중이 내에 액체가 체류(유출)하게 되고, 이로 인해 고막에 가해지는 압력이 증가하게 된다. 증가한 압력은 환자에게 열, 귀 통증, 청력 문제를 야기한다. 급성중이염(AOM)이 재발하는 아동을 치료할 때에는 고막 부분을 작게 절개하고 그 틈으로 고막천공관을 삽입해 중이에서 액체를 빼낸다. 미국에서 매년 약 100만 명에 달하는 아동이 고막천공술을 받는다. 고막천공관을 삽입하면 급성중이염(AOM) 재발률을 줄일 수 있지만, 최근 연구에 따르면 18개월 동안 고막천공관을 삽입한 아동 중 83%에서 한 번 이상 고막천공관 급성중이염(AOMT)이 재발한다고 한다. 해당 아동의 임상 발현은 고막이 온전한 급성중이염(AOM) 아동과 차이를 보이는데, 통증과 열의 빈도가 훨씬 더 낮다. 고막천공관 급성중이염(AOMT)의 가장 흔한 증상은 귀에서 나오는 고름(이루)이다.

용도

OTOVEL은 소아 환자(6개월 이상)에서 황색포도상구균, 폐렴구균, 인플루엔자균, 모락셀라 카타랄리스균, 녹농균으로 인한 고막천공관 급성중이염(AOMT)을 치료할 때 사용된다.

중요한 안전성 정보

사용 금지 사유

다음과 같은 경우 OTOVEL 사용을 금지해야 한다.

- 플루오시놀론 아세토나이드나 다른 코르티코스테로이드, 시프로플록사신이나 다른 퀴놀론 및 OTOVEL의 기타 성분에 과민증을 보이는 환자

- 수두와 단순 포진 감염 및 균류 귀 감염을 포함해 외이도에서 발생하는 바이러스 감염 환자

경고 및 주의

과민증 반응 - 피부 발진이나 기타 과민증 증상이 보이는 즉시 OTOVEL 사용을 중지해야 한다. 전신 퀴놀론을 복용하는 환자에서 첫 사용 후, 심각하고 종종 치명적인 과민증(아나필락시스) 반응이 발생했다는 보고가 있다. 일부 환자의 경우 반응 후 심장혈관허탈, 의식 소실, 혈관 부종(후두, 인두 및 안면 부종 포함), 기도 폐쇄, 호흡 곤란, 두드러기 및 가려움 등이 발생하기도 한다. 심각한 급성 과민증 반응은 즉각적인 응급 치료가 필요하다.

장기간 사용으로 인한 미생물 번식 가능성 - OTOVEL을 장기간 사용할 경우 비감수성 박테리아와 균류가 과도하게 번식할 수 있다. 치료를 시작하고 일주일 내에 감염이 개선되지 않을 경우, 배양균을 확보해 추가적인 치료를 파악해야 한다. 이와 같은 감염이 발생하면 당장 사용을 중지하고, 대안 치료를 시작해야 한다.

지속적 또는 재발성 이루 - 치료 내내 이루가 나올 경우, 또는 6개월 사이에 2차례 이상 이루가 나올 경우에는 추가 평가를 통해 진주종, 이물질 또는 종양 같은 원인을 제거해야 한다.

역반응

임상 시험 경험: 한 명 이상의 환자에서 발생한 가장 흔한 부작용은 이루, 과도한 육아 조직, 이염, 귀 가려움증, 고막 이상, 귀 부종 및 균형 감각 이상이었다.

(Salvat) and Arbor Pharmaceuticals, LLC (Arbor) jointly announce the launch of OTOVEL(R) (ciprofloxacin 0.3 % and fluocinolone acetonide 0.025 %) otic solution, the first and only antibiotic and steroid combination ear drop available in single, sterile, preservative-free, unit-dose packaging. About OTOVEL(R)

OTOVEL(R) (ciprofloxacin 0.3 % and fluocinolone acetonide 0.025 %) is indicated for the treatment of acute otitis media with tympanostomy tubes (AOMT) in pediatric patients (aged 6 months and older) due to S. aureus, S. pneumoniae, H. influenzae, M. catarrhalis, and P. aeruginosa. Two phase 3 multicenter, randomized, double-blind, active-controlled, parallel group trials were conducted to assess the efficacy and safety of OTOVEL compared to ciprofloxacin otic solution and to fluocinolone acetonide otic solution. In total 662 pediatric patients (aged 6 months to 12 years old) with AOMT were included. In both trials, the OTOVEL treatment arms showed significantly shorter times to cessation of otorrhea in comparison to both the ciprofloxacin and fluocinolone acetonide alone arms. In Study 1 and Study 2 the median time to cessation of otorrhea in the OTOVEL group was 3.75 days and 4.94 days, respectively.

OTOVEL(R) is packaged in sterile, single-use vials, with each vial containing a delivery volume of 0.25 mL sterile, preservative-free solution. OTOVEL is promoted and distributed by Arbor and is now available nationwide.

About Acute Otitis Media with Tympanostomy Tubes (AOMT)

Acute otitis media (AOM) is an inflammation of the middle ear space, behind the eardrum (tympanic membrane),that usually results from infection. The infection often results in fluid accumulation (effusion) within the middle ear, increasing pressure on the eardrum. This pressure can cause fever, ear pain, and problems with hearing in affected patients. Children that have recurrent AOM may have tympanostomy tubes inserted through a small incision in the eardrum to help facilitate drainage of fluid from the middle ear. Approximately 1 million children undergo placement of tubes in the United States every year. While the insertion of tubes may help reduce the recurrence of AOM, recent studies report that 83% of children with tympanostomy tubes in place for 18 months will experience at least one episode of AOMT. The clinical presentation of AOMT is different than AOM in children with an intact eardrum in that pain and fever are much less common. The most common symptom of AOMT is fluid drainage from the ear (otorrhea).

IMPORTANT SAFETY INFORMATION FOR OTOVEL

INDICATION

OTOVEL is indicated for the treatment of acute otitis media with tympanostomy tubes (AOMT) in pediatric patients (aged 6 months and older) due to S. aureus, Streptococcus S. pneumoniae, H. influenzae, M. catarrhalis and P. aeruginosa.

IMPORTANT SAFETY INFORMATION

Contraindications

OTOVEL is contraindicated in:

- Patients with known hypersensitivity to fluocinolone acetonide or other corticosteroids, ciprofloxacin or other quinolones, or to any other components of OTOVEL.

- Viral infections of the external ear canal, including varicella and herpes simplex infections and fungal otic infections.

Warnings and Precautions

Hypersensitivity Reactions - OTOVEL should be discontinued at the first appearance of a skin rash or any other sign of hypersensitivity. Serious and occasionally fatal hypersensitivity (anaphylactic) reactions, some following the first dose, have been reported in patients receiving systemic quinolones. Some reactions were accompanied by cardiovascular collapse, loss of consciousness, angioedema (including laryngeal, pharyngeal or facial edema), airway obstruction, dyspnea, urticaria and itching. Serious acute hypersensitivity reactions may require immediate emergency treatment.

Potential for Microbial Overgrowth with Prolonged Use - Prolonged use of OTOVEL may result in overgrowth of non-susceptible bacteria and fungi. If the infection is not improved after one week of treatment, cultures should be obtained to guide further treatment. If such infections occur, discontinue use and institute alternative therapy.

Continued or Recurrent Otorrhea - If otorrhea persists after a full course of therapy, or if two or more episodes of otorrhea occur within 6 months, further evaluation is recommended to exclude an underlying condition such as cholesteatoma, foreign body, or a tumor.

Adverse Reactions

Clinical trials experience: the most common adverse events that occurred in >1 patient were, otorrhea, excessive granulation tissue, ear infection, ear pruritus, tympanic membrane disorder, auricular swelling and balance disorder.
